FIRST TIME VISIT


Our team will guide you in selecting the right treatment plan. We’ll review your medical history, assess personal sexual risks, and conduct tests for HIV, kidney function, Hepatitis B, and liver function. At PULSE CLINIC, we can arrange a sexual health screening and dispense up to 12 bottles during your first visit, offering options like Truvada, Teno-Em, Ricovir-Em, Tenof-Em, Descovy, Taficita, and Tafero-Em.

The entire PrEP process at PULSE CLINIC takes just 30 minutes. You’ll also receive a medical certificate allowing you to travel with your PrEP medication globally.

A PrEP appointment includes consultation, risk assessment, rapid HIV and syphilis tests, and metabolic tests (kidney and liver function, and hepatitis B vaccine levels). Once you book a PrEP appointment, there's no need to schedule separate appointments for HIV or other tests.

Be sure to test for HIV, kidney function, and Hepatitis B before starting PrEP. Also, ensure you haven't had potential HIV exposure (unprotected sex, needle sharing) 14-21 days before the HIV test.

 

 

PrEP REFILL (FOLLOW UP VISIT)

If you decide to continue, we can offer up to a 12-month supply per visit for personal use.

  • Ensure that you are HIV-negative and have normal kidney function to use PrEP safely.
  • Take PrEP daily and avoid sharing it with others who haven't been tested.
  • Make sure You don't have other SEXUALLY TRANSMITTED INFECTIONS.
  • If you have an STI, seek proper treatment and encourage your sexual partner(s) to get tested and treated as well.

WHAT IS PrEP? 


PrEP is a medication regimen designed for HIV-negative individuals to help prevent HIV infection.

Since 2013, PrEP has been available as a once-daily tablet that contains tenofovir disoproxil fumarate and emtricitabine, which can be taken either daily or on demand under medical supervision. The original PrEP from 2013 is now offered under various brand names, including TRUVADA, TENO-EM, TENOF-EM, TAVIN-EM, and RICOVIR-EM, so you can select the one that is accessible and affordable for you.

In 2019, a new form of PrEP was approved for HIV prevention, consisting of tenofovir alafenamide and emtricitabine, which is taken only as a daily dose and under a doctor’s guidance.

This new PrEP formulation has fewer side effects and is better for your kidneys and bone health. It is available under several brand names, such as DESCOVY, TAFERO-EM, TAFICITA, and Generic Tenofovir Alafenamide/Emtricitabine.

When taken correctly, PrEP can reduce the risk of HIV transmission through sexual activity by up to 99%, allowing you to engage with partners more confidently, knowing you have minimal risk of contracting HIV.

of the United States (since 2013) and HIV prevention Guidelines of Thailand (since 2014) provided comprehensive information which suggests us to use daily oral PrEP to reduce the risk of acquiring HIV infection in adults.

Our advice around PrEP is based on the results of 2 European studies. The 2 studies looked at different ways of taking the prevention.


1. PROUD Study1 DAILY DOSE


How to take PrEP daily

  • Take leading dose of PrEP once a day every day ideally same time a day for 7 days (without condomless sex) to provide enough protection level in the rectal tissue (for anal sex) and in vaginal tissue (for vaginal sex - if you have one)
  • You can take it with or without food.
  • Take one pill once a day
  • If you miss one or two pill once a while, don't freak out! you still have enough protection.
  • No one taking the Truvada correctly caught HIV in this study.
  • This is the officially recommended way to take PrEP since 2013!!
  • This is how PULSE clinic recommends guys, girls and transgirl who have sex often to take PrEP for their protection.
    But some people don't have sex very often, they feel once a day everyday is too much and PrEP in western countries is very expensive (that's why they come to get PrEP at PULSE) here comes PrEP on demand.

 

2. Taking PrEP ON DEMAND (2+1+1) IPERGAY Study2


- Take 2 Truvada pills between 2-24 hours BEFORE sex. Then take one Truvada pill every day until 2 days have passed after the last time you had sex.
- This regimen is for ANAL SEX ONLY, and not recommended for vaginal sex. 
- This method is also called 2+1+1 or On-Demand PrEP, it's good for you if you know you might have sex 24 hours in advance:
How to take PrEP on demand

  •  take 2 PrEP pills 2 – 24 hours before sex
  •  take 1 PrEP pill 24 hours after the first dose
  •  take 1 more PrEP pill 24 hours after that
  • Continue taking PrEP until you have taken 2 doses after your last sex.

 

 

It’s crucial not to miss any doses. If you’re having sex for several days, such as over a weekend, continue taking ONE pill every 24 hours until you have 2 days without sex.

Note: This is not recommended for gay individuals who frequently engage in sex or do not plan their sexual activity well, as there is evidence that many people tend to forget to take the third and fourth pills.

 

 

HOW DOES PrEP PROTECT YOU FROM HIV? 


PrEP is up to 99% effective in preventing HIV transmission through sex when taken correctly. When combined with condom use and DOXYPEP, you can feel confident that your risk of contracting an STD or STI is very low!

However, PrEP is only effective if you are HIV-negative, which is why testing is essential before starting it. Your first PrEP appointment will include an HIV test to confirm your status before you receive a prescription.

Since you’re using these tablets to prevent HIV rather than treat it, you don’t have to take them for life; you can use them as long as you feel the additional protection is necessary. This duration could be weeks, months, years, or even short periods when you anticipate having unprotected sex.

For individuals who engage in drug injection, PrEP can also reduce the risk of HIV transmission by up to 70%.

 

HOW TO TAKE PrEP? 


It depends on how frequently you engage in sexual activity and whether you plan your sexual encounters in advance. The Clinical Practice Guideline of the United States (2013) and Clinical Practice Guideline of the United States (2013) offer detailed information recommending the use of daily oral PrEP to minimize the risk of HIV infection in adults. Our recommendations regarding PrEP are informed by the findings of two European studies, which examined various methods of taking the treatment. Both studies demonstrated an 86% reduction in the risk of contracting HIV.

 

 

PROUD Study1 DAILY DOSE
People took a Truvada pill every day. No one taking the Truvada correctly caught HIV in this study. (This is the officially recommended way to take PrEP.)

IPERGAY StudyON DEMAND
Take 2 Truvada pills between 2-24 hours BEFORE sex. Then take one Truvada pill every day until 2 days have passed after the last condomless sex. This regimen is not recommended for vaginal sex. (and not recommended for gay people who enjoy chemsex or do not plan to have sex very well, because there are already pieces of evidence that many people forget to take the third and the fourth pill.)

Using a ‘2-1-1’ dosage timeline – two pills between 2 to 24 hours before sex, one pill 24 hours after the first pill, and a final pill 24 hours later – PrEP is shown to give protection against HIV that is as effective as daily PrEP.

If you're thinking about using PrEP On Demand, make sure you don't skip any doses. If this is likely to happen, talk to your doctor or therapist about taking daily PrEP instead. PrEP On Demand only protects against HIV exposure during anal intercourse. If you believe you may participate in activities such as chem fun (CF), a daily PrEP regimen will better protect you.

 
 


WORRIED THAT THE DRUGS YOU BOUGHT AREN'T REAL?

My partner is HIV positive, but I'm HIV negative. Can I use PrEP?


Yes, it is recommended to take additional precautions, especially if your partner is still contagious. However, if your partner is on HIV treatment and has an undetectable viral load, the risk of transmitting HIV to you is negligible, so taking PrEP may not be necessary in that case.

 

If I use PrEP, Do I still need to use condoms?


Yes. While PrEP protects you from HIV, it does not offer protection from syphilis, gonorrhoea and other STDs. This is where condoms come in – when PrEP is taken together with regular condom use and DOXYPEP, you can be sure that you are at a very low risk of exposing yourself to STDs.

 

How long does PrEP take to become effective?


For daily PrEP to become effective in preventing HIV, the medication has to build up in the body. This is estimated to take up to 7 days for anal sex. Afterwards, the medication has to be continually taken once daily in order to maintain its
effectiveness.

If you’re taking PrEP On Demand, it’s very important that you stick to the ‘2-1-1’ dosage plan for it to be effective. Take note that PrEP On Demand is only effective for anal sex – protection from HIV for any activities other than anal sex can be obtained through daily PrEP.

 

 

What are the possible side effects?


In the first few weeks of starting daily PrEP, some people may complain of headaches, dizziness, fatigue, or gastrointestinal side effects such as nausea, diarrhea and stomach cramps.

These side effects subside over time as your body gets used to the medication. PrEP may affect kidney function in a very small number of people. Blood tests for kidney function may be included as part of your monitoring regime. If you are concerned about side effects or are experiencing them, discuss them with your doctor at your check-up appointments.

 

What should I do if I have side effects from PrEP?


While PrEP is well tolerated by most people, there may be some initial side effects as your body gets used to the medication. If this happens, you may choose to take your daily dose before bedtime, when you can sleep as the side effects wear off. If the side effects are intolerable or last longer than a month, seek immediate advice from your monitoring doctor.

 

Does PrEP have long-term side effects?


The PrEP medicine is believed to have no long-term negative effects. You will still need to work closely with your doctor to detect any potential adverse effects. Although this is uncommon, some persons using PrEP may report impaired kidney and liver function. Your doctor will do a baseline function test, and if the findings reveal that you have impaired kidney and liver function, blood tests will be performed regularly to monitor your condition.

 

If you take medication for other conditions. Can you still use PrEP?


PrEP has been shown not to interact with most medications. However, it’s best to tell your doctor or pharmacist that you’re taking it before you are prescribed other medications.

 

If I drink alcohol and/or use recreational drugs. Can I still use PrEP?


In research, PrEP has been shown not to interact with the use of alcohol or recreational drugs. However, do bear in mind that being under the influence of alcohol or recreational drugs may cause you to miss your scheduled doses, affecting your adherence and decreasing the effectiveness of your treatment.

 

How do I maintain my adherence?


PrEP should ideally be taken at the same time each day to keep a consistent level of the medication in your body. To achieve this, it's important to make taking your pills a routine habit. Consider using alarms or reminders, and carry a spare dose with you for times when you're away from home.

 

What do I do if I miss a dose??


If you miss a dose of daily PrEP, simply resume taking it at your regular time. Missing a dose occasionally will have little effect on the overall effectiveness of PrEP. There’s no need to double up on doses to compensate for a missed one. However, if you’re using PrEP On Demand, it’s crucial not to miss any doses, as not adhering to the 2-1-1 plan will diminish its effectiveness.

 

Can I take my pills with me when I'm traveling?


If you miss a dose of daily PrEP, just continue taking it at your usual time. Occasionally missing a dose will have minimal impact on PrEP's overall effectiveness, and there’s no need to take an extra dose to make up for it. However, if you’re on PrEP On Demand, it’s essential not to miss any doses, as failing to follow the 2-1-1 plan will reduce its effectiveness.

 

What do I have to do if I want PrEP?


Before you can obtain PrEP, you will have to be assessed if you are suitable for it. This is done in a few simple steps at the clinic.
Step 1: Get a blood test done at a sexual health clinic that prescribes PrEP.
They’ll test you for HIV, and liver and kidney function. The results will be returned to you online in one day.
