What is PrEP?


Pre-exposure prophylaxis (PrEP) is an antiretroviral medication designed to prevent HIV infection before engaging in high-risk sexual activities, such as unprotected sex or when a condom fails. When taken correctly, PrEP is up to 99% effective at preventing the transmission of HIV.

 

Who Should Use PrEP?


PrEP is recommended for individuals who wish to lower their risk of contracting HIV. It is suitable for people of all genders who are sexually active, particularly those who:

  • Do not regularly use condoms during sex.

This is GILEAD's TRUVADA pill sold outside of Thailand. Observe the blue color with one side branded "GILEAD," and the other side doesn't have the number "701" on it.

People from developed countries such as the USA, Canada, Australia, Switzerland, The Netherlands, Germany, Hong Kong, and Singapore are used to this look of their Truvada pill.

This is GILEAD's TRUVADA pill sold in Thailand. Observe the lighter/pale blue color with one side branded "GILEAD" as Truvada sold outside of Thailand, but the other side doesn't have the number "701" on it.

Don't freak out when you receive Truvada in Thailand and find out it is light blue without 701. This is not a fake pill. They are legit Truvada from Gilead made in Germany.

This is the RICOVIR-EM pill manufactured by Mylan in India, and sold around the world. Observe the blue color, which is very similar to the Truvada, with one side branded "M117" and the other side doesn't have anything on it.

These pills are tested and approved by the Thai FDA, Ministry of Health. They are being used for treatment to suppress HIV effectively among hundreds of thousands of people living with HIV in Thailand. What Should You Do Before Starting PrEP?


Before beginning PrEP, individuals must undergo medical screening to assess their eligibility. This includes blood tests to check for:

  • HIV infection
  • Hepatitis B infection
  • Kidney function

If the test reveals any issues—such as an existing HIV or hepatitis B infection or impaired kidney function—PrEP will not be recommended for that individual.

 

Are There Any Side Effects of PrEP?


PrEP is generally well-tolerated, but some users may experience mild side effects when starting the medication, such as nausea. These symptoms usually subside within 3–5 days of continued use.

FAQs About PrEP in Thailand


Can you buy PrEP over the counter in Thailand?

No, PrEP is not available over the counter in Thailand. PrEP is a prescription medication that you can get from a healthcare provider after proper evaluation.
 

If I take PrEP, should I stop using a condom when I have sex?

While PrEP is highly effective at preventing HIV, it only offers protection against that specific virus. Unfortunately, it does not protect against other sexually transmitted infections (STIs) such as gonorrhea, chlamydia, syphilis, herpes, or genital warts. If you engage in unprotected sex, these STIs can still be transmitted. If you're comfortable with the potential costs of treating these conditions, you may choose to skip using a condom, but it is still strongly recommended to use one to protect against both HIV and other STIs.


How long do I need to take PrEP?

A typical bottle of PrEP contains 30 pills, which lasts for one month if taken daily. However, there are other dosing schedules for taking this HIV prevention medication. You should consult with your doctor to determine the best regimen based on your individual needs.

 

How long do I need to take PrEP before it is effective?

PrEP reaches full effectiveness after seven consecutive days of daily use. This timeline applies to all individuals, regardless of age or gender. There is also an alternative regimen called the 2+1+1 method (also known as PrEP on demand), which is specifically recommended for men who have sex with men. This method takes effect immediately after the first dose. It’s advisable to consult with your doctor before starting any PrEP regimen.

 

Does taking long-term PrEP lead to harmful health effects?

While PrEP is generally safe, long-term use may affect your liver or kidney function. Regular tests every 3–6 months are necessary to monitor these organs. If test results indicate elevated levels, your doctor may recommend pausing PrEP or switching to a different medication. In rare cases, PrEP may cause a decrease in bone density, and if this occurs, your doctor will likely advise you to stop taking PrEP to allow your bone density to recover.

 

Can you start PrEP after you have been exposed to HIV?

PrEP is designed to prevent HIV infection before exposure. It is not effective for treating HIV after exposure. If you have already been exposed to HIV, taking PrEP may be dangerous, as it can encourage the development of drug-resistant HIV strains, which are harder to treat. For this reason, an HIV test is required before starting PrEP to ensure that you are not already infected.
