Pre-PrEP checklist: How can I get PrEP at PULSE Clinic?


  1. Do you already have the prescription?
    • If you already have a prescription for PrEP, you can show us your prescription and buy the medication.
  2. If not, do you have health check-up results from the last 3 months?
    • If you already have one, you can get a 12-month supply at PULSE.
    • If you do not have recent health check-up results, you can get tested at PULSE and receive a 12-month supply of PrEP.
    • You can also get fewer than 12 bottles and have the rest delivered to your door.

WHAT WILL BE DONE FOR ANY WOMAN GETTING PrEP FOR THE FIRST TIME AT PULSE CLINICS?


The usual to do list for FIRST VISITS* are:

  1. Consultation
  2. Rapid HIV test.**
    • An HIV antibody antigen test have a much shorter window period than other tests – they can give an accurate result 4 weeks after potential exposure to HIV. It’s a good idea to repeat the HIV test after 4 weeks to make sure an acute or recent infection was not missed by the first test.
  3. Hepatitis B virus**
    • to rule out an active hep B infection. The the drugs in PrEP can also suppress the hep B virus and we’d take this into consideration when helping you start or more importantly stop taking PrEP.
  4. Full STI screen including syphilis test
  5. Kidney function tests (In a very small number of people PrEP can damage renal (kidney) function.
    • This is rare and usual only in people with existing kidney problems or taking other medication that may affect their kidneys. But not to worry since there's the new safer PrEP available now!)
  6. PrEP prescription & dispensary right at the clinic***

FIRST VISIT


Our team will help you choose which treatment plan to follow. 
We’ll check your medical history, personal sexual risk evaluation and run HIV test, kidney function test, Hepatitis B and liver function blood tests. 
We can arrange a sexual health screen at our PULSE CLINIC (Bangkok, Phuket, Chiang Mai, Pattaya, Hong Kong, Kuala Lumpur, Singapore, Manila).
We can dispense up to 12 bottles at the first visit. You can chose from;
Truvada, Teno-Em, Ricovir-Em, Tenof-Em, Descovy, Taficita, Tafero-Em

The whole process to acquire PrEP at PULSE CLINIC takes 30 minutes.
You will also receive medical certificate that will allow you to travel with PrEP medications in every countries.

PrEP appointment includes consultation, risk evaluation, rapid HIV and syphilis test, metabolic test (kidney function, liver function and hepatitis B vaccine level). Once you book appointment for PrEP, you don't need to book appointment for HIV test or other tests.

  • Make sure you get tested for HIV, kidney functions, Hepatitis B virus before taking PrEP.
  • Make sure you didn't have exposure to HIV (bareback sex, sharing needles) 14-21 days before HIV test

 

 

PrEP REFILL (FOLLOW UP VISIT)


It’s important to have regular monitoring. We’ll make sure everything is going OK and doing well. 
We’ll
check HIV status, do syphilis test. Twice a year we’ll run a blood test to check your kidneys and liver with doctor, counselor.
And if our doctor ordered your labs in advance you can use our
PULSExpress service to make things faster for you. 

If you decide to continue we can sell you up to 12 month’s supply each visit for personal use.

  • Make sure you're HIV NEGATIVE, have normal kidney function to continue taking PrEP safely.
  • Make sure you take PrEP daily and do not share your PrEP with those who have not get tested.
  • Make sure you don't have other SEXUALLY TRANSMITTED INFECTIONS
  • Make sure you get proper treatment when having STDs and tell your sex partner(s) to get tested and treated too.

I Am A Woman, Can I Consider to Take PrEP?


If you think PrEP may be a good option for you, here are a few questions you can discuss with your health care provider:

  • How often—and for how long (i.e., days, weeks)—do I need to take PrEP if I am trying to protect myself from HIV? What happens if I miss a dose or several doses?
  • What are likely side effects and how can I manage them?
  • How often will I need to be tested for HIV?
  • How much will the drug cost me? Will it be covered by my insurance? Will the HIV tests also be covered by my insurance?
  • Do any of my current medical conditions make PrEP drugs not a good choice for my health overall?
  • Do any of my other prescription medications, over-the-counter drugs, street drugs, herbs, vitamins, or supplements have interactions with the drug?
  • What should I do if I become pregnant while on PrEP?

While PrEP is designed to be used in combination with condoms or other safer sex measures, women who cannot or do not use internal condoms may want to use PrEP instead of condoms. It is important to understand the overall risks and benefits of using PrEP to reduce one's HIV risk, as well as to consider methods of protection from other sexually transmitted infections or diseases (STIs or STDs).

 

Starting PrEP: how long it takes for PrEP to protect you?


Women need to take PrEP every day for 7 days to be protected against HIV.

 

HOW TO TAKE PrEP? 


If you have vaginal sex we recommend Daily PrEP.
This is the same for:

  • cis and trans women
  • neo-vaginal sex for some trans women.

If you have receptive anal sex only there are more dosing options. We recommend daily PrEP for all trans people using hormone treatment as we don’t have sufficient data to support other dosing options.

It depends on how often do you have sex and  do you plan to have sex well in advance?
Clinical Practice Guideline of the United States (2013) and HIV prevention Guidelines of Thailand (2014) provided comprehensive information which suggests us to use daily oral PrEP to reduce the risk of acquiring HIV infection in adults. Our advice around PrEP is based on the results of 2 European studies. The 2 studies looked at different ways of taking the treatment. Both of the studies showed an 86% reduction in catching HIV. 

PROUD Study1 DAILY DOSE
People took a Truvada pill every day. No one taking the Truvada correctly caught HIV in this study. (This is the officially recommended way to take PrEP.)

IPERGAY Study ON DEMAND
Take 2 Truvada pills between 2-24 hours BEFORE sex. Then take one Truvada pill every day until 2 days have passed after the last condomless sex. This regimen is not recommended for vaginal sex. (and not recommended for gay people who enjoy chemsex or do not plan to have sex very well, because there are already evidences that many people forget to take the third and the forth pill.)

 

When PrEP might benefit you?


PrEP will not be the best choice for everyone but it is worth thinking about if:

  • condoms are not easy to use or not always used when you have sex
  • you’ve recently had a vaginal or rectal sexually transmitted infection (STI)
  • you’ve recently had PEP (post-exposure prophylaxis)
  • you have sex with people whose HIV status you don’t know
  • you have sex with people who are HIV positive but not known to be virally supressed/undetectable
  • you have sex with people from communities that are disproportionately impacted by HIV
  • you have sex in cities or countries outside the UK where HIV affects a high proportion of the population.

You do not need PrEP if your only sexual partner is HIV positive with a undetectable viral load. They cannot pass on the virus even without condoms.

 

Stopping PrEP – temporarily or permanently


You can decide to stop taking PrEP whenever you want. But don’t stop taking the pills on the day you decide.

You need to keep taking your daily dose for 7 days after your last risk of exposure to HIV – condomless sex, for example.

 

Starting PrEP again


You can choose to start taking PrEP again whenever you want.

You will need:

  • an HIV test if you’ve had a risk of exposure – condomless sex for example – since stopping PrEP
  • to take a daily dose for 7 days before relying on PrEP to protect you from HIV. This allows full protection to build up in your system.

 

PrEP and contraceptives, conception and breastfeeding


When you take PrEP, you CAN:

  • Take hormonal birth control – ‘the pill’. It is safe and both will work if taken correctly.
  • Get pregnant. PrEP prevents HIV, not pregnancy.
  • Conceive and carry a baby. It is safe for you and your baby.
  • Breastfeed safely.
  • Take feminising hormone therapy safely and effectively if you are a trans woman. PrEP will not stop the hormones working, or cause fat redistribution in the body or face.
 

WORRIED THAT THE DRUGS YOU BOUGHT AREN'T REAL?
